Report of Illness
Please note: You must be following standard company policy by calling in to the restaurant 651-222-5676 and by submitting this form at least 3 hours prior to your scheduled shift.
If you are testing positive for COVID, this will begin a cycle wherein we will need you to stay away from the restaurant for the health and safety of all;
with symptoms = minimum 5-day leave, without symptoms = minimum 3-day leave. In either case, you must NOT have a fever in order to return to work.
If you are requesting use of your PTO benefit, please also complete that form, a link will be provided in the confirmation of this report.
